Funding
HCPH receives funding form a wide variety of public and private sources including:

• Medicare, Medicaid
• Private Insurance
• VA
• Elderbridge Agency on Aging
• United Fund
• Private donations
• Client fees
• State and federal grants
• County Tax dollars

We do not turn clients away if they are unable to pay or need services.

Availability

Referrals or requests for service may be made by patients, staff, doctors, hospitals, clergy, friends, or family members by call our office 1-515-832-9565 or 1-800-873-1473 during regular office hours. On call 24 hours a day.
