HOSPICE CARE

 

Hospice services are provided to eligible clients in Hamilton County.  The program is Medicare certified.  Hospice is a special way of caring for people with a terminal illness.  A team of professionals and trained volunteers provide care to the patient and the family.  They’re dedicated to providing comfort and relief from pain.  Hospice is designed to help you make the most of each hour and each day of remaining life.

                   Home Care Aide                      Occupational Therapy

                   Homemaker                             Speech Therapy

                   Spiritual Coordinator              Bereavement Coordinator

HOSPICE QUALIFYING CRITERIA

 

·        Client must be certified terminally ill by your physician

·        Request Hospice Care

·        Care is focused on palliative care

·        There is a primary caregiver or family member to provide the patient’s day to day needs
