RN: Specializes in Admissions and Home Visitation.
Home visitation for adults, admissions.

RN: Specializes in Public Health Programs: immunizations, flu clinic, communicable disease follow up. TB follow up, bio-emergency planning,  environmental inspections, home visitation.

RN, Maternal Child Coordinator: Specializes in Pediatric Nursing, Healthy Opportunities for Parent to Experience Success (HOPES), breast feeding, Early Head Start, maternal health programs.

RN: Specializes In home visitation across the life span.  HOPES through adults, WIC program, and Communicable Disease follow –up.



CDA:  Specializes in Infants Children and Families through Early Head Start and HOPES.

SW:  Specializes in infants children and families through HOPES, Early Head Start and Homemaker Admissions.

Case Manager:  Specializes in case management for frail elderly, follow up on bereavement issues, and Tobacco Grant.

Diane, Amanda, Marilyn, Lindsay:
Office and financial support, presumptive eligibility for Medicaid, Hawk-I, Meals on Wheels applications, Water, Well, and Septic Programs

Mandi, Rena, Estefanie, Kim:
Direct Care Workers providing personal care, light house keeping, light meal prep.

Sanitarian:  well, septic and water program.
